IL State Health Department has a new requirement for 9th grade students. Please read below for details.

Click here for the  "Proof of Dental Examination" or "Dental Waiver".

• All students in Grade 9 need a dental examination by May 15th 2021

• Dental examinations can be dated anytime between November 15, 2019 and May 15th 2021 to satisfy this requirement.

• Proof of Dental Examinations needs to be submitted to LGHS anytime between now and May 19, 2021

• If dental care is unavailable, one can submit a Waiver
