Step 2
Medical Portal
This will require medical information and the following documents: Insurance Cards, IEP (if applicable) and Birth Certificate.
IL State Health Form and immunizations, Eye Examination Form and Dental Examination Form will be required upon acceptance.
These forms are all available to print and upload in the medical portal.

Direct Documents
Report Cards: Must arrive directly from school. Please click here for Transcript/Report Card Release Form, if your school is requesting you send them one.
Can be submitted through the following methods:

Academic Testing and ​Interview
Once you have completed the above steps, our admissions team will carefully review your application. Applicants who show alignment with our school will be contacted to continue the application process, which will include an in-person interview and academic testing. An invitation to this stage does not guarantee acceptance.

Tuition and Fee Information
Tuition and fee information can be found by accessing our school website tuition tab . Scholarship information is available upon request. Admission is contingent upon approval from the Board of Directors.
